Calaveras Street and Hospital Discharge Program to Support the Unhoused Thanks to $190,000 Grant from Health Net

Health Net is proud to support initiatives that help increase access to healthcare. We recently awarded a $190,000 grant that will help Calaveras County Public Health provide streamlined healthcare and housing support for people and families who are struggling with homelessness. With our support, they will launch the Calaveras Street Health and Hospital Discharge Pilot Program.
The program will do two things:
- Deliver medical care and essential services directly to those living in unsheltered environments
- Provide navigation support for local housing options that can temporarily house Medi-Cal members transitioning out of hospital care.
Calaveras County is leading the effort with help from local partners, including Sierra HOPE, Calaveras County Health and Human Services Agency, Mark Twain Medical Center and the Mark Twain Healthcare District, with additional support from Anthem and CommonSpirit Health.
Getting quality healthcare in rural areas, especially for people experiencing homelessness, requires creative and collaborative solutions. Health Net is proud to team up with Calaveras County Public Health and local groups to provide essential resources so we can work toward real progress in health.
