California Primary Care Providers Gain Practice Transformation Support Through Multi-Health Plan Partnership

Primary care is the backbone of our healthcare system. It helps keep people healthy, prevent illness and manage long-term conditions. That's why three major California health plans—Health Net, Aetna and Blue Shield of California—are joining forces to make primary care stronger.

Through the California Advanced Primary Care Initiative's Payment Model Demonstration Project, we're giving doctors better tools, data and resources to deliver better care. The goal is simple yet powerful: transform community health by improving how primary care works. Specifically, this project focuses on tackling key health issues, like managing diabetes and blood pressure — two of the most common and serious conditions in the U.S.

This collaboration was built from the ground up, in partnership with the California Quality Collaborative and the Integrated Healthcare Association. Together, we developed payment models, performance metrics and strategies to help ensure success at every step.

To support participating provider organizations, the project offers:

  • Extra funding to adopt the care model
  • Expert guidance to support practice transformation
  • Ongoing coaching to implement changes
  • One place for all providers to view patient data
  • Value-based care incentives tied to better patient outcomes

When health plans align their efforts, big things happen. By empowering primary care providers with the right resources, we can improve outcomes and make a real difference for patients and communities.
